Enclose in Your Envelope:
  1. The Motor Vehicle/DNR Registration Renewal(s).
  2. A Check or Money Order for the Correct $$ Amount. Your Driver License
    Number and Phone Number MUST be Included on Your Check to Ensure Delivery
  3. Make Sure Your Address is Correct.
  4. Include a Self Addressed Stamped Envelope
  5. Verify This is the Correct Vehicle
  6. It is Your Responsibility to Verify all Information is Correct to Ensure
    Renewals are Processed and Delivered.
Any fraudulent statements subject the application to a fine up to $700.00. Returned checks
are subject to a $30.00 NSF fee. All dishonored checks will constitute criminal charges.
Criminal penalties can include a loss of driver license and/or motor vehicle license. The
issuant can also be sentenced to 90 days in jail and/or $700.00 fine, plus vehicle impoundment
and fees incurred from such impoundment.
